RESUMO

La afectación ovárica como debut de un linfoma de Burkitt sin enfermedad extraovárica detectable es anecdótica, por lo que habitualmente no se incluye como hipótesis diagnóstica tras el hallazgo de una tumoración ovárica. Su desconocimiento lleva a realizar un tratamiento equivocado que puede llegar a comprometer el deseo reproductivo de la paciente. Presentamos el caso de una paciente que presenta un linfoma de Burkitt con afectación ovárica como manifestación inicial. La paciente desarrolló una progresión sistemática excepcionalmente rápida. A propósito de este caso y de su inusual evolución, revisamos la literatura existente.


Ovarian involvement as the initial manifestation of a Burkitt lymphoma without detectable extra-ovarian disease is rare, which is why it is usually not included in the differential diagnosis when an ovarian tumor is detected. A missed diagnosis will lead to the wrong treatment being given, and this can compromise any future reproductive wishes of the patient. In this article, a patient presents a Burkitt lymphoma with ovarian involvement as an initial manifestation and an unusually rapid systemic progression of the disease. Prompted by this case and its unusual course, we reviewed the existing literature.

RESUMO

Objetivos: Descripción de una complicación posquirúrgica infrecuente, la evisceración intestinal vaginal, y revisión de los factores predisponentes, diagnóstico y su tratamiento quirúrgico corrector definitivo, las vías de abordaje y las posibles técnicas quirúrgicas, basándonos en la literatura científica disponible. Material y métodos: Se presenta el caso clínico de una paciente de 46 años con la complicación referida, su evaluación inicial, manejo quirúrgico y seguimiento. Se realiza laparotomía para evaluación de la viabilidad de las asas intestinales herniadas, reparación del defecto existente en la cúpula vaginal y técnica quirúrgica correctora en el mismo tiempo quirúrgico, la colposacropexia con malla con buen resultado a corto y medio plazo. Conclusiones: La evisceración vaginal en una complicación infrecuente que requiere una valoración rápida y una actitud quirúrgica urgente, debido a la alta morbimortalidad a la que se asocia


Objectives: We describe an uncommon postoperative complication, vaginal evisceration of the intestine, and review the predisposing factors, diagnosis and definitive corrective surgery, the surgical approaches and possible surgical techniques, based on the available scientific literature. Material and methods: We report the case of a 46-year-old patient with the aforementioned complication, initial evaluation, surgical management and monitoring. We performed a laparotomy to assess the viability of the herniated intestinal loops, repair the defect in the vaginal vault and perform a surgical correction in one procedure. We used a colposacropexy mesh with good short- and medium-term results. Conclusions: Vaginal evisceration is a rare complication that requires rapid assessment and urgent surgical treatment due to the high morbidity and mortality with which it is associated

RESUMO

El ectima gangrenoso es una manifestación dermatológica característica de una infección severa causada casi siempre por Pseudomonas aeruginosa (P. aeruginosa), con o sin bacteriemia. Las infecciones severas por P. aeruginosa se presentan habitualmente en pacientes con inmunodepresión u hospitalizados, en los pacientes sanos existe el antecedente de foliculitis y forunculosis. El diagnóstico es evidentemente clínico con confirmación en cultivos. El ectima es una manifestación cutánea de una infección severa que lleva a confusión diagnóstica y causa retraso en el inicio del tratamiento. El tratamiento quirúrgico precoz asociado a una cobertura antibiótica amplia y a un soporte metabólico y nutricional adecuados son la primera elección terapéutica. Se presenta el caso de una paciente joven inmunocompetente que presentó un shock séptico asociado a neutropenia, ectima gangrenoso e infección de vías urinarias causado por P. aeruginosa


Ecthyma gangrenosum is a dermatological manifestation characteristic of a severe infection almost always caused by P. aeruginosa, with or without bacteraemia. Severe infections by P. aeruginosa usually present in immunocompromised or hospitalised patients, or in healthy patients with a history of folliculitis and furunculosis. The diagnosis is clinical with culture confirmation. Ecthyma is a cutaneous manifestation of a severe infection that leads to misdiagnosis and delayed treatment. Early surgical treatment associated with a broad antibiotic coverage and adequate metabolic and nutritional support are the first therapeutic choice. We describe the case of a young immunocompetent patient who presented septic shock associated with neutropenia, ecthyma gangrenosum and urinary tract infection caused by P. aeruginosa

RESUMO

La piómetra tiene una baja incidencia (0,2-0,5%) y es más habitual en mujeres de edad avanzada. Aparece cuando existe una estenosis del orificio cervical y es más frecuente su asociación a lesiones malignas (1,5-4%), aunque en ocasiones puede ser benigna o idiopática. Se han señalado como causas predisponentes la incontinencia urinaria y el cáncer de colon. Su diagnóstico es radiológico y es importante la sospecha clínica. El adecuado manejo incluye antibioterapia de amplio espectro y drenaje de la colección por vía vaginal y, en casos seleccionados, la histerectomía. La complicación más grave e infrecuente sería la perforación o rotura uterina, con tasa de mortalidad significativa. Se presenta el caso de una mujer anciana institucionalizada con pluripatología, hospitalizada por leucorrea y mal estado general secundario a infección urinaria de evolución tórpida, con el hallazgo ecográfico de colección uterina compatible con piómetra que se maneja con drenaje evacuador periódico y antibioterapia intensiva prolongada, con buena evolución y estudio de extensión negativo para malignidad. Realizaremos una actualización en el manejo del piómetra


Pyometra has a low incidence (0.2-0.5%) and is more common in older women. This infection occurs because of cervical stenosis. More frequently it is associated with malignant lesions (1.5-4%), although the cause can sometimes can be benign or idiopathic. Urinary incontinence and colon cancer have been identified as predisposing causes. Diagnosis is radiological and clinical suspicion is important. Correct management includes broad spectrum antibiotic therapy and drainage of the collection through vaginal access or hysterectomy in selected cases. The most serious and rare complication is uterine perforation or rupture, which has a significant mortality rate. We report the case of an elderly institutionalized woman with multiple comorbidity, hospitalized for leucorrhoea and poor general health status secondary to a torpid urinary infection. Ultrasound revealed a uterine collection compatible with pyometra. The patient was treated with periodic vaginal drainage and prolonged intensive antibiotic therapy. Outcome was favourable and the extension study was negative for malignancy. We provide an update of the management of pyometra

BACKGROUND: The vulvar cancer is the fourth more frequent neoplasia after the endometrial, cervix and ovarian cancer. Normally, it has been related to old women of ages from 70 to 80 years old. Rarely, it has been detected cases in adult or young women. However, its incidence has been increased in the last years and in more early years. It is for this change in the incidence and its appearance in early years why a possible etiology has been looked for, opening different hypothesis that go from that related to the HPV to those that study an inflammatory chronic process as the basis for the carcinogenesis. CLINICAL CASE: In this article, it has been presented the case of a woman who is 34 years old with negative VPH that made her debut with epidermoid carcinoma of the vulva moderately different and on purpose of the case, we do a revision of the literature existent. CONCLUSIONS: Vulvar cancer diagnosed in young women as in older, but with different trends, risk factors and natural history. The case reported here escapes the theories studied so far so needed new lines of inquiry to investigate this form of presentation young woman, without HPV infection.
